Participation

I grant permission for my child to participate in SuperD! Friends ,a program of SuperDville LLC. – educational programming for children who learn differently.

Release and Waiver of Liability.

In consideration for my child’s participation in the programs, activities, and events organized by SuperDville, LLC , I, on behalf of myself, my child, and our respective heirs, executors, administrators, assigns, and personal representatives, hereby voluntarily and knowingly assume all risks of participation. I fully and forever release, waive, discharge, and covenant not to sue SuperDville, LLC its directors, officers, employees, agents, contractors, representatives, successors, and assigns (“Released Parties”) from any and all claims, liabilities, demands, causes of action, damages, losses, or expenses (including attorneys’ fees and costs) arising out of or related to my child’s participation in the Programs.

I further agree to indemnify, defend, and hold harmless the Released Parties from and against any and all claims, demands, actions, suits, damages, liabilities, losses, costs, and expenses (including attorneys’ fees) brought by me, my child, any member of my family, or any other person, arising out of or related to my child’s participation in the program, SuperD! Friends, owned by SuperDville, LLC..
I understand and acknowledge that by signing this Release and Indemnification Agreement, I am giving up substantial legal rights, including the right for myself, my child, or my family members to sue the Released Parties. I sign this voluntarily and with full knowledge of its significance.

I hereby release SuperDville, LLC, its employees, representatives, and assigns from any and all claims, demands, or causes of action that I may have against them by reason of anything contained in the video or the use thereof.

This release shall be binding upon me and my heirs, legal representatives, and assigns. I agree to the terms of this release.

Video Release

I grant permission to SuperDville, LLC (owner and creator of SuperD! Friends curriculum and programming) to use my child or my likeness and or voice, as recorded for the purpose of a blog post, and promotional material and I understand that his/her and my image and or voice may be used in various forms of media, including but not limited to, digital, print, online, and broadcast.
I waive any right to inspect or approve the finished product wherein any of the aforementioned likeness appears. Additionally, I waive any claims to royalties or compensation arising from the use of the video.

I understand that SuperDville, LLC is the sole owner of all rights in and to the video, and they may use, reproduce, distribute, and display the video without any further consent from me.
